Child Support Worksheet Calculator (Massachusetts Department of Revenue Website) *Use this worksheet to calculate the presumptive amount of child support to be ordered by the Court. Financial Statement (Short Form) * If your Gross Income (before taxes & deductions) is less than $75,000/yr, the Court requires you to complete and file the Financial Statement (Short Form) if you are the Plaintiff, Defendant, or Petitioner in a Divorce, Separate Support, Paternity, Modification or Contempt case or any other case involving alimony, child support or division of property. If you are self employed, you must also complete schedule A. If you own any rental property, you must also complete schedule B.
